Vampire Wario is a counterpart of Wario who appears as a character in Dr. Mario 64 and its port included within Nintendo Puzzle Collection. Unlike Wario Land 3, in which Vampire Wario is the name of a transformation, the one in Dr. Mario 64 is both a standalone character and a transformation of Wario's.

In Dr. Mario's story in Story mode, if the player has not lost in Normal difficulty or higher, Wario eats the Megavitamins at the end, causing him to transform into Vampire Wario. When Wario is defeated, Vampire Wario is unlocked as a playable character.
As a computer-controlled opponent Vampire Wario is one of the characters with the highest star difficulty, shared with Rudy and Metal Mario. For his losing animation, Vampire Wario transforms into a bat, but in his winning animation, Vampire Wario cackles.
