They have the appearance of red Question Blocks with three yellow spikes above the question mark symbol. Spiked Question Blocks repeatedly rotate 180 degrees. If Mario or Luigi hit or Ground Pound any side without the spikes, the Spiked Question Block turns into an Empty Block.

Spiked Question Blocks only appear in one level, World 6-3. There are only four Spiked Question Blocks, three of which contain a power-up item.
