Dr. Mad Scienstein is a character who has taken on a various occupations throughout his appearances, most notably as a scientist, and he has varied between being a supporting character or an enemy. Mad Scienstein first appears in the Japan-exclusive game Kaeru no Tame ni Kane wa Naru, in which he is named Arewo Shitain-hakase. He was later inducted into the Mario franchise in Wario Land 3, in which he is an enemy, and made a reappearance in Dr. Mario 64. In both games, it assists Rudy the Clown.

Wario becomes invisible as a result of Mad Scienstein's potion in Wario Land 3
Mad Scienstein's only main attack is throwing magic potions at Wario to turn him invisible, allowing him to go through security gates. Unlike many other enemies in Wario Land 3 which were transformed into creatures by Rudy, Mad Scienstein appears to have not been transformed in any way, due to his human-like appearance.
In Dr. Mario 64[]
Rudy wants Mad Scienstein to cure his cold so that Mad Scienstein must steal some Megavitamins to cure his cold. Dr. Mario, along with Wario, follow them and fight various enemies from Wario Land 3 until reaching Mad Scienstein and his boss Rudy. They manage to defeat them and cure Rudy's cold. Mad Scienstein is also playable in this game.

- Mad Scienstein is not a harming enemy and is more of an ally, due to turning Wario invisible. This changes in Dr. Mario 64 when he is the main cause of the story problem.
- His name is a portmanteau of Mad Scientist and Einstein or Frankenstein.
